General Information

Office hours:
Our office is open Monday-Friday from 8:30 a.m. to 5:00 p.m. For all after hours emergencies call hospital emergency room for assistance.

Insurance:
We accept most major insurance providers & need you to bring your current insurance card to all appointments at the Fayetteville Pain Center.

Payment Options:
We accept cash, check, Visa, Mastercard & American Express. At the time of your visit you will be expected to pay any co-payment, deductible, or other expenses not covered by your insurance

Appointments:
Seen by physician referral only. If you cannot keep your appointment at the scheduled time, please try to give us 48 hours notice. Failure to furnish 48 hours notice of cancellation may result in a $25.00 no-show charge.

Emergencies:
Contact the hospital emergency room or 911.
